Nous avons pour cela créé une unité de télésoins spécialisée, qui développe et fournit aux professionnels de santé des outils et services leur permettant de garantir un niveau de prise en charge élevé pour les patients.
Satelia est un acteur de santé qui permet aux hôpitaux, cliniques et médecins libéraux de faciliter la prise en charge « hors les murs » des patients atteints de maladies chroniques. Pris en charge à 100% par la sécurité sociale, Satelia leur apporte les outils digitaux et humains, ainsi que les données pour répondre à la réalité de leurs besoins de terrain.
Notre souhait est d’aider nos collègues soignants à améliorer la qualité de vie de leurs patients, ainsi que leurs connaissances de leur maladie; et ainsi participer à les rendre acteurs de leur santé.
Satelia aide depuis 2018 au suivi des patients ayant une insuffisance cardiaque, et forte de son expertise, propose depuis peu des solutions de prise en charge en cancérologie.
Satelia repose sur 3 piliers : humain, scientifique, logiciel.
La plateforme humaine lutte contre toute exclusion numérique et permet de réaliser le suivi des patients en situation d'illectronisme, la coordination des soins, et l’accompagnement thérapeutique nécessaire à l’augmentation des connaissances du patient. À ce jour plus de 55 infirmières formées au télésuivi et à l’accompagnement thérapeutique agissent en ce sens, avec une écoute soignante experte.
