Créée en mai 2011, SOINS COORDONNES est une association loi 1901.
Elle accompagne les professionnels de santé dans la mise en œuvre de projets contribuant à une prise en charge coordonnée et pluriprofessionnelle des patients.
L’association promeut la coordination et la coopération comme principes d'organisation du système de santé pour répondre à un triple défi, médical (explosion des maladies chroniques, vieillissement...),
professionnel (évolution de la démographie médicale, diminution du nombre des installations...)
et financier.
Fortement impliquée dans le paysage professionnel, SOINS COORDONNES réunit 14 professions de santé (médecins généralistes, infirmiers, biologistes, pharmaciens, kinésithérapeutes, podologues, orthoptistes, opticiens, sages-femmes, dentistes, psychologues, diététiciens, ergothérapeutes et orthophonistes).
Enfin, SOINS COORDONNES promeut l’indépendance des professionnels de santé ambulatoires et l’accès à des soins de qualité pour tous. Sa finalité demeure avant tout la reconnaissance de chaque professionnel de santé en tant qu’acteur de santé.
