L’APTA 78 est une association créée en novembre 2018, financée par l’ARS et le Conseil Départemental, à la gouvernance plurisectorielle et pluriprofessionnelle regroupant les acteurs du secteur sanitaire et médico-social du département des Yvelines.
L’APTA 78 répond aux besoins des professionnels libéraux afin de faciliter la prise en charge de leurs patients :
- Accompagner les professionnels pour faciliter leur exercice et coordonner leurs interventions avec celles des structures de santé
- Piloter et déployer des projets innovants sur l'organisation des parcours de soins et les modalités d'exercice des professionnels
