    We are convinced that personalized medicine, predictive thanks to artificial intelligence, will sustainably improve the situation of elderly people at home or in an institution.

    PRESAGE is developing the first medical device that predicts the risk of emergency hospitalization of an elderly person living at home within 7 days.

    The result of 4 years of clinical research involving the National Agency for Health Performance (ANAP), 2 ARS, AP-HP and CNAV, this device for healthcare professionals is the first medical device based on artificial intelligence to anticipate emergency hospitalizations.

    PRESAGE is made up of around ten employees and a scientific council comprising professors of medicine, e-health experts and data science researchers.

    Its clients are, on the one hand, guardianships (ARS, CD) and local authorities who wish to improve home monitoring of at-risk patients in a territory and, on the other hand, home help structures (independent living residences, SAD, etc.) that use PRESAGE to improve the maintenance of people's autonomy and avoid trips to the emergency room.

    L’ensemble de la planète connait un vieillissement de la population humaine. L’avance en âge est caractérisée par plusieurs phénomènes dont la fréquence et le retentissement croissent avec l’âge : fréquence des maladies chroniques, concomitance de ces maladies chez les mêmes individus (polypathologie), dépendance fonctionnelle et besoin d’aide dans les actes de la vie quotidienne, recours à l’hospitalisation et au placement en institution gériatrique.

    Nous sommes convaincus que la médecine personnalisée, prédictive grâce à l'intelligence artificielle, va améliorer durablement la situation des personnes âgées à domicile ou en établissement.

    PRESAGE développe le 1er dispositif médical qui prédit à 7 jours le risque d’hospitalisation en urgence d’une personne âgée vivant à domicile.

    Issue de 4 années de recherche clinique impliquant l’Agence nationale de la performance en santé (ANAP), 2 ARS, l’AP-HP et la CNAV, ce dispositif à destination des professionnels de santé, est le premier dispositif médical basé sur l’intelligence artificielle pour anticiper les hospitalisations en urgence.

    PRESAGE est composé d’une dizaine de salariés et d’un conseil scientifique regroupant professeurs de médecine, experts en e-santé et chercheurs en data science.

    Ses clients sont d’une part des tutelles (ARS, CD) et des collectivités territoriales qui désirent améliorer le suivi à domicile de patients à risque d’un territoire et d’autre part, des structures d’aide à domicile (Résidence autonomie, SAD...) qui utilisent PRESAGE pour améliorer le maintien de l’autonomie des personnes et éviter des passages aux urgences.
